An unexpected start-up of equipment or machinery can cause serious, even fatal, injuries. This type of accident usually occurs during maintenance operations or modification of the machinery. To prevent such accidents, a thorough lockout/tagout program is required. Lockout/tagout involves placing a lock and/or a tag on the controls or energy isolating devices of a machine in accordance with an established lockout/tagout program.
