Abstract
A recording form for museum acquisitions is proposed. The form would be designed to ensure that museum staff personnel acquire all the information that the donor may be able to supply. Problems in designing such a form are (1) to decide what information is required and (2) to frame questions so that desired information is obtained. A recording form differs from a catalogue card in that the former records information about an object provided by the donor while the latter contains information provided by the museum.
